Students new
to Western School District should enroll prior to the opening of school at
the appropriate building. At the time of registration, the following
information must be presented for each child:
1. Proof of Identity - the child's birth certificate
2. Student's Social Security Number
3. Proof of Residency
4. One of the following:
a. A list of immunizations and record of
dates, signed by a physician, that the child has been immunized
against diseases specified by the Director of Public Health.
As
required by the Public Health Code Section 920 regarding Student immunization
Status, the Jackson County Health Department has instituted a first day
exclusion policy for the 2000-2001 school year. All new entrants to the school
district must present proof of minimum of one dose each of MMR, DPT, and oral
polio vaccine prior to admission. Students who have begun the primary
series within the last month would be considered provisional and
would not be excluded from the first day of school.
b. A statement signed by a parent
or guardian to the effect that the child has not been immunized because
of religious convictions or other objection to immunization. This must be approved by
the local health department prior to enrollment.
5. A statement showing that a vision and hearing test has been
passed is required unless a parent indicates, in writing, that the child cannot submit to
the test because of religious convictions.
6. If transferring from another school district, the name and address of
the school last attended.
New Legislation: Beginning with the year 1997, all students
entering day care must be
immunized against Hepatitis B. Beginning with the year
2000, all students entering
school must be immunized against Hepatitis B. Beginning
with the year 2003, all
students entering school must be vaccinated with the
varicella vaccine (chicken pox). |
