(A maximum of ten (10) days may be approved per year, subject to individual student’s attendance record.)

TO THE STUDENT:

  1. This form is to be completed by each of your classroom teachers. It indicates that they are aware of a planned absence and asks for their opinion whether this pre-arranged absence request should be granted.
  2. When completed, it is to be signed by your parents of guardian and returned to the attendance office five school days in advance of the absence (anything less then five school day advance notice runs risk of not being approved). A parent signature cannot be on this form prior to all teachers filling it out. Teachers should not sign this form if a parent signature is already on it.
  3. If this form is not completed, signed and returned in advance of the requested date(s), your absence may not be completely excused. (Remember, anything less than five school day advance notice runs the risk of not being approved.) In that case, the absences would count toward the five (5) days permitted in the regular policy.
  4. A copy of this completed form will be on file in the attendance office.

 

TO THE PARENT/GUARDIAN:

The staff and administration of the high school are concerned about the academic welfare of your student. We urge you to read comments made by the teachers regarding your student’s upcoming absence from school. We ask your help and cooperation in encouraging your student to make up any work missed during the period of time he/she will be absent. Also, realize that if a teacher disapproves of the absence, it is probably because it will have an adverse affect on his/her grade. Read the teacher’s comments very carefully so that you know if there will be consequences for allowing the student to take the absence. Administration will approve these absences provided the proper procedure is followed and that the parent/guardians have signed this form showing they are aware of any consequences this absence may have on the student’s grade.
